How do I get my Duke chart?
In order for your child to gain access to their own MyChart account, as the parent or legal guardian you must first obtain a Proxy Account for your child by speaking to staff in the office of your child’s provider or by contacting Duke Customer Service at 919-620-4555 or 800-782-6945 between 8:00am-5:00pm ET Monday.

How do I get MyChart activation code?
Go to the MyChart Request an Activation Code page to get your activation code. Or you can ask the receptionist at your next visit to email you a code or go to the Member Services desk and ask to be emailed a code. Or after your visit you will receive an After Visit Summary.

You can sign up for a MyChart account on your healthcare provider’s MyChart website or in the MyChart mobile app. Click Access MyChart at the top of this page to find your MyChart provider, then click Sign Up to request an activation code.
